Re-Certification Criteria:

Option A

1. Have maintained your membership in a LAP chapter during the previous 3 years.

2. Have earned a total of 30 LAPH’S

3. The Commission must approve any other hours of training before a new certificate is issued.

4. All documentation of hours must be mailed with application

Option B

1. Retake the LAP-C test
